COVID Screening Information *Before you arrive*
Effective Sept 13, 2021
The Township of Zorra has implemented a QR code screening assessment at each facility. All visitors to the facility will be asked to provide a positive result to staff at the entrance of each building.
WEBSITE: You can log into the web address: click here OR
QR CODE: Access to the screening tool can be obtained by scanning the QR code with your phone. This code is posted at the door of the facility.

Once the questions have been answered, show your positive result to the facility staff at the door:
OTHER NOTES:
The positive result screen should be valid within two hours of your arrival to the facility.
- One screening can be completed for each Household.
- You may use your phone to complete a screening on behalf of member of another household if they are not able to complete it on their own.
- If you do not have a phone, you will be able to complete the screening at the facility with their available devices.
